  • Heart Transplantation and Deceased Organ Donation – Gujarat’s Story – Dr Dhiren Shah, Director, Heart & Lung Transplant Program Marengo CIMS hospital

Heart Transplantation and Deceased Organ Donation – Gujarat’s Story – Dr Dhiren Shah, Director, Heart & Lung Transplant Program Marengo CIMS hospital

Dr Dhiren Shah, Director Heart & Lung Transplant Program and Surgeon Marengo CIMS hospital

Ahmedabad (Gujarat) [India], April 11: India’s thoracic organ transplant program has evolved over the years and is currently the number one program in the South Asian region. Global coverage of heart transplant is 1.06 PMP (2016-18). About 8,000 to 9,000 heart transplants are performed worldwide each year, and about 50 percent are performed in the United States. South and South-East Asia perform less than 5% of global heart transplants, and India performs more than 90% of these surgeries. The same is true of lung transplantation. More than 7,000 lung transplants are performed globally, of which more than 40 percent are performed in the USA alone.

Dr Dhiren Shah, Director, Heart & Lung Transplant Program Marengo CIMS hospital, “India’s progress with heart and lung transplantation in the last decade reflects the growth of the deceased organ donation program in the country. Currently, there are very few cardiac units in the country who have invested significant time and resources in this program. The growth and momentum has been made possible by the local champions who have done this tireless work. Although the growth has been largely confined to private sector hospitals, the progress has been truly remarkable despite all the logistical and financial challenges.”

Heart transplantation has been an excellent treatment modality for advanced heart failure in the modern world for the past 40 to 50 years. Ten years later the number of heart transplants performed worldwide has increased by more than 300% and in the US by more than 400%. The number of heart transplants increased rapidly after the 1980s in Western countries, particularly in North America, reaching about 3000 to 4000 heart transplants annually. Heart transplants have reached high levels due to the increase in the number of organ donations.

The first successful heart transplant in India was performed in 1994 at the All India Institute of Medical Sciences in Delhi. However, it was not until 2010 that the number of heart transplants increased. In fact, in January 2020, India achieved the milestone of 1000 heart transplants. Almost 25 years after the Human Organ Transplant Act passed by the Government of India. It is quite clear that the number of cadaveric organ donations in India is very low compared to the number of organs required for transplantation. It was only after 2010 that organ donation gradually increased especially in South India and at the same time heart transplants started in the southern states. However, the number of heart transplants in western India is negligible. Nevertheless, in some cities, especially cities like Surat, Bhavnagar and Rajkot in Gujarat, the number of organ donations was continuously increasing.

In 2016, Marengo CIMS Hospital, Ahmedabad got a heart transplantation license and Gujarat’s first heart transplantation was performed on 19 December 2016. Due to heart transplantation, which is undoubtedly the most famous and prestigious transplant procedure, awareness about organ donation has increased exponentially to every comer of the state.

Between 2017 and 2019, there has been a nearly 20% increase in deceased organ donation in Gujarat. Marengo CIMS Hospital was still the only heart transplant hospital in Gujarat and following the turnaround the numbers were steadily increasing. When Covid-19 hit in 2020, the spirit of organ donation was still high. Even amid the lockdown, Gujarat conducted India’s first cadaver organ donation during the Covid pandemic where hearts, livers and kidneys were procured. Deceased organ donation in Gujarat doubled from 36 organ donors in 2020 to 70 in 2021 and increased from 110 to 223 in 2020 and 2021 respectively.

The Gujarat model adopts a multi-pronged approach to the deceased organ donation programme, which includes:

    • Government initiatives, policies
    • Dedicated and committed support from the medical community
    • Unremitting work and persistence from NGOs
    • Media collaboration to spread awareness about organ donation
    • Government hospitals and medical colleges take initiatives to increase organ donation

If these measures are taken into consideration, it will not take long to reach the number of organ donations of 10 per million populations. Which is currently 1 per million populations in India. With the increase in organ donation, there is a need to have a fair and transparent organ sharing policy and this will be possible through organizations like the National Organ and Tissue Transplant Organization (NOTTO).

The support of reputed organizations like Institute of Kidney Disease and Research Center (IKDRC) and non-governmental organizations (NGOs) has helped in reaching these heights. It so happened that in recent years Surat became the city with the highest number of organ donors in the country, but surprisingly, the city did not have a single heart transplant centre. The number of heart transplants may be a key indicator of the status of deceased organ donation in a particular state or country. The number of heart transplants seen in Tamil Nadu, Kerala and Andhra Pradesh in early 2010 and later in Gujarat and Maharashtra clearly shows that it can act as a catalyst to increase cadaveric organ donation in the country.

Ahmedabad’s Civil Hospital, one of the largest hospitals in Asia, hardly had any organ donation until 2020. Although 30 to 40 patients are on ventilators in the neurosurgical ICU at any given time, a large number of brain-dead potential organ donors cannot be found. In 2020-21, the state government launched an organ donation campaign at the civil hospital with the support of non-governmental organizations, which resulted in a significant increase in the hospital’s cadaveric organ donation. Civil Hospital, Ahmedabad has donated 100 of its deceased organs in a period of two years including 77 livers, 152 kidneys, 23 hearts and 18 lungs.

Despite a severe second wave of Covid-19, Marengo CIMS Hospital, Ahmedabad performed 14 heart transplants in 2021, of which 9 were performed in the last two months of the year. In fact, three heart transplants were performed in a span of 6 days in December 2021. The dream run of organ donation in Gujarat continued in 2022, reaching the magical figure of 100 deceased organ donations in a span of eight months and declaring Gujarat as one of the states with the highest number of deceased organ donations in the country.

  • The Role of Homeopathy in Integrated Medicine

The Role of Homeopathy in Integrated Medicine

    World Homeopathy Day

New Delhi (India), April 11: India is facing a huge gap in the present healthcare system. According to the National Health Mission, India is undergoing a significant healthcare shift, with the incidence of Non-communicable Diseases (NCD) exceeding the burden of Communicable Diseases. Cancer, heart disease, chronic respiratory difficulties, diabetes, and other Non-communicable diseases account for 4.1 million fatalities, accounting for more than 60% of all deaths. Homeopathy offers a safe option for the treatment of NCDs with no side effects.

    Today’s Healthcare Challenges

    • Insufficient funds

The spending on healthcare is the lowest in India. It is even lower than our neighboring countries.

    Country % of GDP
    China 6.7
    Myanmar 4.68
    Nepal 4.45
    Pakistan 3.38
    Bangladesh 2.5
    India 2.1

The annual healthcare budget of India is 5,96,440 Crore.  With a population of 141.73 crore, only 23.76 Rupees are spent per person / per year on healthcare.

Because healthcare spending in India is inadequate, it should be supplemented with a less expensive medical system. Homeopathy fits the bill.

    • The workforce is not sufficient

India’s doctor-patient ratio is the lowest among the developed and developing countries in the world.

    Country Doctor- Patient ratio
    Russia 5: 1000
    Germany 4: 1000
    Australia 4: 1000
    France 3: 1000
    China 3: 1000
    United States 3: 1000
    Brazil 2: 1000
    India 0.5: 1000

A single doctor in India serves at least 1,456 individuals, which is much higher than the WHO standard of one doctor for every 1,000 people.

Community Health Centers (CHCs) in India lack 83.2 percent of the required surgeons, 74.2 percent of the required obstetricians and gynecologists, 79.1 percent of the required physicians and 81.6 percent of the required pediatricians. Overall, there is a 79.5% shortage of experts at CHCs.

There are 5 lakh homeopathic doctors, with 2 lakh homeopathic doctors practicing actively and 3 lakh not practicing, a majority of whom are women. By using telemedicine in conjunction with a work-from-home strategy these could be employed to treat NCDs and reduce the gap of qualified doctors required in the country. This will add 40% to the existing medical health work force in our country. It will also prevent unnecessary hospitalizations.  By using  homeopathy in the treatment of NCDs, we can bring down healthcare costs and increase national productivity.

Use Homeopathy to keep a young work force healthy

As a country becomes older, the expense of supporting an aged population rises.

    Country Average working age
    Japan 48 years
    China 37 years
    India 23 years

Fortunately, India has a highly productive population of young working people.   According to the Human Resources for Health Study, roughly two-thirds of all Indian workforce are under the age of 40, with more than 25% being under the age of 30.It is imperative that to increase national productivity we keep our youth healthy and fit.  However, on an average 16.4 working days per year / per worker are lost due to illness.  This often increases due to the side effects of chemical medicine, self-treatment and antibiotic misuse. Homeopathy as a safer system of medicine with no side effects helps to increase productivity of a young work force.

The vision of a ‘Bimari Mukt Bharat’ can be achieved through Homeopathy with Government support.

    Inadequate Insurance coverage

Individuals in India lack adequate insurance coverage. A study entitled “Health Insurance for India’s Missing Middle” produced by NITI Aayog states that almost 45 million Indians lack financial protection for health.

In accordance with the research, the Ayushman Bharat scheme covers 50% of the population. Around 20% of the general population is covered by social and private voluntary health coverage. The remaining 30% of the general population receives no monetary support for healthcare treatment as they are not poor enough to be covered by government-subsidized schemes and yet not rich enough to buy private health insurance – this group is known as the ‘missing middle’.

Country % of the population insured by the government
    China 95%
    France 95%
    Germany 85%
    United States 78%
    India 50%

By covering homeopathy through insurance the cost of premiums will come down.  The consumers pay out will be less and the coverage can be increased.  Hospitalisation is the third leading cause of debt in India.  Using homeopathy as preventive medicine can save costs of hospitalisation. Let me give you two examples:

    1. If diabetes is treated in the early stages with homeopathy, it can prevent chronic kidney disorders (CKD) and save costs of dialysis and hospitalisation later.
    2. Similarly, if high cholesterol can be brought down by homeopathic medicine, one can save lacs on heart treatment and hospitalisation.

Import foreign patients and export foreign doctors

Around two million individuals worldwide from 78 countries travel to India every year for healthcare, well-being, and IVF treatments, generating $6 billion for the business, which is predicted to grow to $13 billion by 2026 under the government’s Heal in India plan.  Homeopathy should be included as part of Heal in India.  India has the largest number of homeopathic doctors in the world (5 lacs) who have great clinical experience due to the large number of patients that they treat.  Using their expertise, promoting homeopathy through Indian embassies abroad and issuing AYUSH visas for homeopathic treatment will hugely increase medical tourism in India.

India has over 250 homeopathy colleges affiliated to universities.  In many countries abroad a 6 month certification course in homeopathy allows foreign doctors to practice homeopathy in their country.  If Indian universities had a special abridged homeopathy course either on their own or outsourced through a public / private venture, we could attract a lot of foreign students to India.

After training they would go back to their country and propagate homeopathy learnt in India thereby, truly making India a“Vishwa Guru”.

Over the years, while rehabilitating people with disabilities, Sansthan has closely seen the miseries of a large number of Divyangs who have lost their limbs in accidents. It was then thought that large camps should be organized and around 3500 amputees would be benefitted from modular prostheses and restart their stalled life. To fulfill this goal, we approached strong and affluent gentlemen of society. These social workers present today understood the pain and suffering of people with disabilities and fulfilled the resolution of the organization in just 60 days. For which we consider it our pride to honor them.

Welcoming all the award winners, the Sansthan President said that Narayan Seva Sansthan was started by his father Padmashree Kailash ‘Manav’, and mother Kamla Devi in the year 1985. The Sansthan has so far done free surgeries for more than 4,35,000 specially-abled people. The high-quality, light weighted Narayana limbs manufactured by the Sansthan have been fitted to more than 35,000 individuals with disabilities who were rendered limbless due to amputation of their arms and legs in an accident. Not only this, about 1 lakh families were saved from starvation even in Corona. We are providing accommodation, food, medicine, and treatment facilities to about 3 to 4 thousand Divyangs and their families daily. The organization has helped 15 million people directly or indirectly. This is the power for the organization to work with satisfaction and service. Sansthan Director Vandana Agarwal joined in thanking the guests, award winners, and their families.
