  • Silent sufferers: It’s time to talk about Pelvic Pain

    Pelvic pain often goes undiagnosed, correct diagnosis is the key to treatment

     New Delhi [India], April 26: Pudendal nerve is one of the main nerves in the pelvis and is closely involved with genital sensation, urinary and bowel functions. Irritation or damage of this nerve often presents with pain and/or altered sensation in the genital, rectal region or deep inside the pelvis.

    Mimicking a plethora of symptoms, the patients may present to several different specialties including urologists, neurologists, colorectal surgeons, gynaecologists, general surgeons, and general physicians, but the problem often remains undiagnosed due to low awareness about the condition. Conditions with overlapping symptoms involving different specialities often fall through the hoop, with no single speciality able to take ownership and address all components of the problem.

    “Similar was the case with a 24-year-old male who presented with severe pelvic pain for the last 18 months. He had been having severe difficulties in sitting for more than 5 minutes and gave his exams lying down. The initial symptoms started with penile pain and burning sensation that gradually progressed to the testicular, buttock, and rectal pain. This was accompanied by the feeling as if he has a urinary infection with burning, difficulty emptying the bladder, and poor urinary flow. Although he had problems sitting he felt much better when sitting on a toilet seat.” Said Dr Amod Manocha, Senior Consultant & Head, Pain Management Services, Max Hospital, Saket

    Due to the lack of specific tests, the diagnosis of this condition remains a challenge. Investigations are requested to support the diagnosis and rule out other conditions which may present similarly. These may include special MRI scans and nerve tests. Resolution of pain after a nerve block, even if temporary, supports the diagnosis of pudendal neuralgia. Pudendal Neuralgia is known to be associated with many other conditions such as dysfunctional voiding, painful bladder syndrome, Chronic Pelvic Pain Syndrome, chronic prostatitis etc.

    “Despite visiting many specialists, trying many medications there was no relief. All my investigations were coming back as normal and some specialists even suggested that it was all in my head. Convinced that there is a problem and unable to find a cause, I started doing internet research and made extensive notes for almost three months before suspecting that the source of my problem may be related to the Pudendal nerve. Then started the journey of trying to find a specialist who can confirm my suspicion and help me, finally I found Dr Amod Manocha and decided to take a trip with my family to Delhi for a consultation.” Said the patient

    “History, examination and investigations pointed towards the suspicion of Pudendal neuralgia, and this was further strengthened by the relief from the nerve block. This young man subsequently underwent radiofrequency treatment of the pudendal nerve and this helped improve his sitting ability, penile pain and urination difficulty. Following the treatment, he was able to sit and have a meal with my family after a long time. This may be a routine thing for most of us but was a big milestone for him. The pain and other symptoms reduced substantially with an overall improvement of about 65% compared to the pre-treatment levels. He is a very motivated patient and easily adopted the lifestyle modifications such as using a coccyx cushion while sitting, avoiding constipation, tight clothes, and bike riding. His compliance with physiotherapy has been good and now that the symptoms are better controlled we are slowly reducing his medicines intending to come off them.” Added Dr Manocha

    Management of this condition requires active patient participation and the use of a multidisciplinary approach. Input from different specialists such as urologists, colorectal surgeons, gynaecologists are often required with a team approach to evaluate and address different components of the symptoms.  A multidisciplinary approach involves using a combination of lifestyle changes, medications, physiotherapy, psychological techniques, and pain-relieving interventions such as injections. Lifestyle changes are aimed at protecting the nerve and reducing its irritation. Combining these with other modalities mentioned previously provides the patients with the best opportunity of having a successful outcome.

    Once he was back home, he locked himself for over 10 hours, and as the holy spirit-led and guided him, he penned the final lyrics and melody of the gospel. “The first lines I penned were Aasma Se Aaye Hai Koi Saugat, Aur Main Karta Hu Jaijaikar (From heaven has come a gift and I sing praises),” humbly shares Kamle.

But people hardly know how the water they consume reaches them. How it is contaminated during the treatment and supply stages etc.

    “The density of population in urban India is quite high and so is the scarcity of safe drinking water. Therefore, the majority of the population consumes water from privately owned water sources. In such a situation, knowing the way drinking water reaches them, turns very important considering its safety and purity. Usually, due to the hectic schedule, people living in urban India don’t notice the discrepancy in the water pipelines of their own houses, which more often causes water contamination. So, it is obligatory for an individual to have a close eye on its maintenance. There are other sources of contamination that people usually don’t pay heed to such as the water tank placed on their roofs if not cleaned at regular intervals contaminates the water stored in it,” says Dr. Prof. Sanjay Rai, Professor, Dept. of Community Medicine, AIIMS, President, IPHA.

    “Also, in urban India, Reverse Osmosis (RO) is one of the most effective water treatment processes being practised and widely used for water purification by the majority of the population. It is generally used for home water treatment to remove salts, chemical toxins, organic contaminants, dyes, pesticides, and microbes. In this process, the raw water is forced (with pressure) through a dense membrane filter that prevents the passing of impurities. Although, the Reverse Osmosis process has certain benefits such as it does not produce or use any harmful chemicals, yet water consumed after the treatment through the RO process is devoid of useful minerals. Unless minerals are added back, the RO water could have harmful effects on health. Therefore, it is very important that the water you drink must contain the right minerals to maintain the balance of the body,” added, Dr. Rai.

    In India, access to treated piped water continues to be inadequate for the majority of the population. Water from major sources like tube wells and hand pumps is also found to be unsafe as they are known to be carriers of waterborne diseases. While people blame unhygienic food for most of the ailments, waterborne diseases such as cholera, acute diarrhoeal diseases, typhoid, and viral hepatitis have caused 10,738 deaths, over the last five years since 2017. Of this, acute diarrhoeal diseases have caused maximum deaths followed by viral hepatitis, typhoid, and cholera. [2]

  • Public Affairs Forum of India (PAFI) announces New Office Bearers for 2021-22. Dr. Subho Ray to be the new President

    New Delhi [India], April 23: Public Affairs Forum of India (PAFI), (www.pafi.in) is a professional resource for the public, corporate affairs, and public advocacy.  PAFI provides a platform and opportunity to exchange information, share knowledge and experiences in engaging with various stakeholders like the government, media, industry associations, multilateral agencies, and think tanks.

    PAFI today announced its new office bearers for the year 2021-22.

    Dr. Subho Ray took over as the President of PAFI. Subho Ray is president of the Internet & Mobile Association India (IAMAI) since March 2006. He heads the permanent secretariat of the association and is an ex-officio member of its governing council.  Before joining IAMAI, Subho was Director for the ICT vertical at the Confederation of Indian Industry in New Delhi, where he worked since 1998, closely with the software & services, BPM, hardware, and telecom industries.

    “It is a moment of great pride to be the president of India’s largest association of public policy and public affairs professionals. PAFI would like to focus on working with the industry, government, and civil society in three specific areas: the overhaul of the health care sector with lessons learned during the pandemic; ease of doing business, especially for emerging sectors such as information & biotechnology and electric vehicles; and participation in and supportive of global and regional value chains especially in electronics manufacturing,” said Dr. Subho Ray.

    Dr. Subho Ray takes over the role of President, PAFI, from Mr. Ishteyaque Amjad, upon completion of his successful term.

    “In the most fateful year in our lives, despite all the challenges, we at PAFI could accomplish a lot. Throughout the pandemic, our member companies continued to make contributions to the communities, we partnered with governments and civil societies to save lives and support livelihoods. During the year, we hosted the highest number of PAFI dialogues, a truly global Annual National Forum, and started the Annual Lecture Series. Despite being physically apart, we came closer as a fraternity. I am sure that under Subho’s leadership, PAFI will scale greater heights.” said Mr. Ishteyaque Amjad, outgoing President PAFI and Group Head – Corporate Affairs, Godrej.

    Mr. Virat Bhatia, Managing Director, Apple India, who leads strategy & policy for the company in India, is the new Vice President of PAFI. Mr. Tanmoy Chakrabarty Group Government Affairs Officer, Tata Sons Pvt. Ltd., is the new Secretary of PAFI.  Ms. Vinita Sethi, SVP & Chief Public Affairs at the Apollo Hospitals Group, is the new Treasurer of PAFI.

  • Hon’ble Minister of Commerce and Industry, Shri Piyush Goyal assures support to Services Exporters

    New Delhi [India], April 23: The Hon’ble Commerce and Industry Minister, Shri Piyush Goyal held an export performance review meeting yesterday and applauded the Export Promotion Councils for their respective sectors’ export performance.

    Shri Maneck Davar, Chairman, Services Export Promotion Council(SEPC), the only export promotion Council mandated to promote Services exports from India, highlighted the contribution of the services sector and the trade surplus in services despite the challenging times and negative growth (April20-Feb 2021) as per RBI data.

    The services sector reeling under the massive disruptions caused by the COVID pandemic since last 2020 was hoping to show its resilience to bounce back strongly is now finding the clock turning back due to the recent surge of a far more aggressive covid wave. Chairman, SEPC underlined the decline in exports of key services sectors like travel and tourism, medical value tourism, education, and aviation. He mentioned that the travel and tourism sector, a major contributor to exports has seen a decline of about 73% with a loss of employment and immensely grim prospects of any sort of business continuity in the near future.

    Chairman, SEPC made a fervent appeal to Hon’ble CIM to consider the persistent representation of services sector exporters to issue the release of SEIS notification for 2019-20 which would partially alleviate working capital needs of the distressed services sector. Shri Maneck Davar, further added that SEPC had suggested that the Department of Commerce and Industry could consider the release of SEIS amounting to previously disbursed amount to bring some relief to the services sector. Ministry of Commerce could consider capping as well in view of spreading the cover of limited funds to all cross-sections of the services exporters.  SEPC has already submitted a proposal on the alternative to SEIS for inclusion in the proposed FTP. The chairman mentioned that in view of the extension of the existing FTP till September 2021, Government should consider giving SEIS for 2019-20.

    Shri Piyush Goyal in his remarks appreciated the contribution of the services sector in India’s export and mentioned that the Services Sector has demonstrated immense resilience and the decline in services exports is only down by 5% since last year’s figures. He assured all necessary support to the services sector in their efforts to tide over the crisis. He asked EPCs to take up the issues pertaining to other Ministries while assuring all necessary support. He advised SEPC to organize the interaction with the Ministry of Medium and Small Industries, Ministry of Finance, and Ministry of Commerce and Industry to look into the issues and support required by the services sector more specifically travel and tourism, healthcare (medical value tourism), education and aviation. Shri Goyal mentioned that he would be pleased to meet large and small exporters to collectively draw mitigation measures.

    SEPC will be undertaking all necessary efforts to sustain the business continuity, revival, and global networking in its endeavour to serve the services sector and meet the export target set by Shri Goyal for the current year.
