  • Around 85% of diabetes-related amputations start with foot ulcers: Dr. Arun Bal

    Mumbai, December 14: Diabetes has evolved as a major healthcare challenge in India. In 2019, India had 77 million people living with diabetes. The numbers are expected to rise to 101 million in 2030 and 134 million in 2045. Diabetes is associated with multiple complications such as nerve damage and poor blood supply, making the foot more prone to ulcers. Diabetic foot ulcers have a tendency to get worse pretty fast if not treated early.

    Dr. Arun Bal, Consultant Diabetic Foot Surgeon, Raheja Hospital, says, “High blood glucose is associated with multiple healthcare disorders, and foot ulcers are one of the common complications of uncontrolled diabetes. Diabetic foot ulcers are stubborn and can result in amputation if not treated in time. Around 85% of diabetes-related amputations start with foot ulcers. In India, the awareness around the condition is poor, and people often try to manage the ulcers. They often reach late to the doctor, and there is no other option than amputating the limb.

    Speaking on one of his patients, Dr. Bal said, “A 64-year-old male with a history of Type-II Diabetes for eight years presented with right toe swelling and ulceration for three months. Each time this swelling occurred during this period, he ignored it as the affected region lacked any sensation. A few days later, he had another episode of swelling of the same toe and presented to his family physician with redness, edema and extensive ulceration at the same region. He was then referred to me by his physician. After a complete examination, amputation of the right toe was suggested by me. The toe could have been saved if the patient had consulted me earlier.”

    The treatment of diabetic foot ulcers has evolved with the latest researches and newer molecules. From the times of world war, tincture iodine was the treatment of choice for most ulcers, diabetic or non-diabetic. The evolution of iodine ensured that better-acting iodine is now available for the management of ulcers. Cadexomer iodine is the latest form of iodine used to manage most diabetic feet. This molecule ensures that patients do not have to go daily for dressing change, thereby reducing the financial burden on the patient. Additionally, Cadexomer Iodine has shown to be effective in treating ulcers, thereby reducing the requirement of surgical debridement.

    Dr. Bal added, “In the case mentioned above, the timely intervention of the right treatment could have helped us avoid the amputation of the affected toe. Cadexomer Iodine has proven to be the dressing of choice for such complicated wounds in diabetic patients due to its safe and prolonged duration of antiseptic action.”

  • Finding the right physician is the first step to cure blood diseases

    New Delhi (India), December 13: Bone Marrow Transplantation (BMT) from a donor (Allogeneic BMT) is possibly the most challenging procedure in modern medicine, which has evolved over the last 50 years, curing millions of patients suffering from blood diseases. Both malignant (leukemia/lymphoma/myeloma) and non-malignant (thalassemia/sickle cell anemia/aplastic anemia/ primary immunodeficiencies) however, the success of BMT is incumbent upon finding successful immunological compatibility between the patient and the donor- called matching of HLA or immunological signature, which is inherited from the parents and varies from person to person. There is a 25% chance of two siblings being completely HLA-matched with each other, which basically means that 3 out of 4 patients requiring a BMT would not find a compatible donor.

    The question often asked is why an unrelated third person cannot be the donor! The answer lies in the fact that the immunological signature or HLA is extremely variable, and the chance of 2 random persons to match with each other is less than one in a million.  While this problem has been largely addressed in Europe and America, for the Caucasian people, with the creation of large unrelated donor registries, where 3 out of 4 Caucasians can find a completely HLA-Matched Unrelated Donor (MUD). This is large because of the very conserved genetic pool of the population, which is not true for a country like India, where thousands of different ethnic races inhabit. The solution for such patients lies in using HLA half-matched or Haploidentical family donors (not unrelated). Haploidentical BMT is one of the most difficult forms of BMT, and no Indian centre performed such transplants in 2010, when Dr Suparno Chakrabarti first initiated Haploidentical BMT program in India.

    Dr Suparno Chakrabarti, an alumnus of PGI, Chandigarh, who initiated the Clinical Hematology program at the institute in 1995, returned to India in 2010. He had left for the UK in 1998 and spent 12 years in both clinical and basic research in the field of BMT, initially as a Research Fellow and subsequently as a faculty. Several paradigm shifts happened in the field of BMT during this time, and Dr Chakrabarti made an immense contribution to the field with several path-breaking research. However, his yearning to contribute to his own country brought him back to India. The last ten years were no less significant in terms of his contribution to the field of BMT and immunology. Dr Sarita Rani Jaiswal joined him in 2011 as a trainee at a premier healthcare centre in Kolkata, where the first BMT Centre of eastern India was created.

    Dr Chakrabarti had an enormous task in hand, having initiated the most difficult type of BMT for the first time in India, with little support from the institution and otherwise. He was criticized by many at that time for indulging in such a high-risk procedure. But the initial cohort of patients all had very advanced blood cancers without any other treatment options. Dr Sarita Jaiswal trained under him for 3 years before going abroad and training under the pioneers in the field of Haploidentical BMT in Italy and Seattle, USA.

    In view of an indifferent attitude of administration, colleagues and the Bengali population overall, Dr Chakrabarti decided to move his base to Delhi. Dr Sarita Jaiswal, being true to the mission of her mentor, joined him at Dharamshila (Narayana) Hospital in 2014, where Dr Suversha Khanna, the founder-promoter of the hospital and Mr Anil Kumar, the outgoing President of the trust, helped them create a true ecosystem for flourishing in both clinical and basic research.

    With only 10-20% survival for Haploidentical BMT in 2012-13, Dr Chakrabarti and Dr Jaiswal invested in extensive research along with the clinical work in Delhi. The rest is now history. Currently, in over 150 Haploidentical BMT carried out between 2015-2021, the survival stands at over 80% in both malignant and non-malignant disorders. Dr Jaiswal had made oral presentations every year between 2015-2021 in scientific conferences in Europe and USA. Their innovations in the field of Haploidentical BMT had researchers in the USA adapt their approach. While this has not been an easy journey, the will, dedication, and honesty paid off at the end.

    Both Dr Jaiswal and Dr Chakrabarti rue about the fact that only a handful patients are benefitting from their path-breaking work. Most of the country remains ignorant about the scientific breakthroughs which are happening in their own backyard. They do not have the academic, financial, or political patronage to leverage their achievements for the benefit of the millions who need it. Despite all these limitations, they have not given up. In the memory of his late mother, Dr Chakrabarti started the MANASHI CHAKRABARTI FOUNDATION, which was aimed at supporting research and treatment for children with Blood Diseases. They have initiated a Comprehensive Care Centre for Thalassemia and Sickle Cell Anemia at Dharamshila Narayana Hospital. They have been joined in their endeavour by Dr Nivedita Singh, an administrator by profession, who had been a part of their journey through the rough times. Many young doctors have also joined hands to form a platform called BLOODSRUS, under the auspices of MCF, where they are ready to provide comprehensive service for blood diseases to all those who have been deprived of such care until now.   • Dr. Uday Tamhankar from Ceramco Dental Clinic Honoured With COVID-19 Warrior Award by Times Applaud

    November 30: The world went through a nightmarish phase in the past couple of years due to the COVID-19 pandemic, but there were some really great personalities who worked pretty hard to help others in critical times. Though the majority of us had the privilege of staying indoors with our family members, people with dental issues suffered a lot during this period because all the dentists had no option other than closing their clinics. The reason behind it is the fact that wearing masks was one of the biggest necessities in order to stop the spread of the virus, but in a dental clinic, the mask is the first thing that needs to be removed for treating a patient.

    Dr. Uday Tamhankar, the founder of Ceramco Dental Clinic, which is India’s biggest private clinic, was continuously getting contacted by those who were having dental problems, and he gave a free consultation to each person who contacted him. Dr. Uday Tamhankar helped people like this for around 40 days, but then he strongly felt that something more needed to be done as many of his patients had been suffering a lot because of dental problems and taking antibiotics for a long period can also have a pretty adverse effect on their health.

He provided his car and driver free of cost to his staff so that they would be safe from public transport. And he and his wife seldom travelled by rickshaws.

    Dr. Uday Tamhankar is such a noble soul that he gave heavy discounts to some of his patients as he knew that jobs and businesses had been affected adversely in the times of pandemic, and he even treated some people for free who were left with no source of earning. It’s also highly commendable that he paid his staff full salary despite his own earnings being reduced to one-fourth in the COVID-19 pandemic.

    Dr. Uday Tamhankar is certainly an inspiration for not just people of the medical fraternity but all of us as he kept the welfare of mankind above everything else and didn’t even care about his own life. It would be apt to say that people like him keep humanity alive in these times when it feels that people are just concerned about themselves.

  • Nearly 25% Patients Are of Less than 50 Years Old out of 18 Lakh Stroke Cases In India Every Year: ISA

    Indian Stroke Association is celebrating October as ‘Stroke Awareness Month’ to create awareness about stroke

    October 23: The prevalence of stroke is on the rise in India, with as many as 18 lakh stroke cases being reported in India every year, according to the Indian Stroke Association (ISA), which has launched several initiatives to create greater awareness about stroke.

    The ISA observes October as Stroke Awareness Month and has organised dozens of programmes and events to spread general awareness about stroke and reduce its burden. An extensive social media campaign with the hashtag #ProtectYourBrain – KnowAboutStroke has been running since October first week.

    “Stroke is a brain attack and is the leading cause of disability and second leading cause of death in India. One person has a stroke in India every 40 seconds, and one-fourth of the patients are less than 50, including a large number who are in the age group of 19-30,” said Dr. Jeyaraj Pandian, President, ISA. who is also Vice President of World Stroke Association.

    “Stroke can happen to anyone, at any age and anywhere. It is estimated that one in every four persons will have a stroke during their lifetime. The risk can be reduced by regular checks for hypertension, diabetes, hyperlipidemia, etc. Exercising regularly, maintaining the right weight, quitting smoking, and giving up alcohol consumption can also help reduce the risk of stroke. Primary Prevention, Acute Treatment and Stroke Rehabilitation are key comprehensive treatments of stroke,” said Dr. Arvind Sharma, Secretary, ISA.

    The theme of ‘World Stroke Day 2021’ is focused on Saving Precious time by raising awareness about the signs of stroke, treating stroke as a medical emergency and the need for timely access to quality treatment. The ISA has run various social media campaigns for Know your numbers, Know your risks, recognise stroke symptoms, Act in time, among others, as a part of Stroke Awareness Month.

    At 19-24% in most states, the stroke fatality rate in India is much higher than in developed countries due to lower awareness about stroke prevention and treatment.

    However, the fatality rate in North-Eastern states and coastal states is as high as 42%, among the highest in the world, which is attributed to higher salt intake.

    “In the North East, many people have salted tea, and in coastal areas, the consumption of dried fish, which has a lot of salt, increases the risk for hypertension, which in turn contributes to higher stroke risk,” said Dr. Pandian.

    As a part of Stroke Awareness Month, which will culminate on the occasion of World Stroke Day on October 29, the ISA has taken the lead in public awareness campaigns through risk screening camps, audio-visual presentations for the public, community engagement through social media and other platforms, collaborations with Indian red Cross Society, Rotary Club, and other NGOs, and a host of other activities throughout the month.

    The ISA has lined up several more awareness initiatives that will continue well beyond Stroke Awareness Month.

    ISA has also collaborated with the Indian Council of Medical Research, National Institute of Epidemiology, Chennai and Indian Hypertension Control Initiative (IHCI) to conduct nationwide webinars to educate doctors, nurses, paramedical staff, and other frontline healthcare workers at the primary level in 100 districts. The activities are focused on highlighting various aspects related to stroke prevention, treatment, and recovery.

    “In the event of a stroke, recognising the symptoms and reaching the hospital in time is critical. There are many effective therapies for stroke if administered within the golden period of 4.5 hours of getting the stroke. During a stroke, every minute 20 lakh neurons die, and hence the need to Act FAST” informed Dr. P. Vijaya, Executive Committee member, ISA.

    In collaboration with the Indian Medical Association, ISA is organising a webinar for the medical fraternity about stroke management on October 29.

    Stroke cases put a tremendous financial and social burden on personal, family, and community levels, but awareness is the key to reducing the burden. A sedentary lifestyle, smoking, obesity, and lack of regular exercise are the key factors that lead to stroke. It is estimated that 80% of stroke cases can be prevented with lifestyle and behavioural changes.
