    Respiratory disease burden highest in India, COPD one of the fastest growing respiratory diseases: SMSRC

    New Delhi (India), July 16:   SMSRC & Group (Medclin Research and Synaegis), one of India’s leading healthcare research, intelligence & consulting organizations has published findings on major disease trends prevailing in India, and how they are currently being managed by healthcare providers. The study reveals that Diseases of the Respiratory System as the largest disease area in India.

    The study was conducted on the basis of MAT Feb 2024 prescription audit – de identified and aggregated data, of more than 10,700 doctors over a 15-year period. Drs in the sample were distributed across all key specialties, across 80+ cities/towns of India. This is a first of its kind real world evidence-based study from an Indian context.

    According to the publication, Diseases of the Respiratory System is the largest disease area in India, constituting 15.3% of all Rx of total patients with diseases in India. Endocrine & Metabolic diseases (e.g. Diabetes and others) is at second place with 13.5% share of Rx of all patients, followed by GI related diseases and circulatory diseases (hypertension, cholesterol etc) as the next ranked disease area.

    Respiratory diseases that constitute of various upper/lower respiratory tract diseases, are treated via the following therapeutic class of medicines – antibiotics, cough & cold preparations, pain relievers and respiratory products. Respiratory products constituting of anti-asthma tablets, syrups, inhalers and nebulizer categories are one of the fastest growing therapeutic class of medicines in India. 

    Further, within Diseases of the Respiratory System, URTI (Upper Respiratory Tract Infection) constitutes 27% of the share in 2024 and is the largest type of respiratory diseases. However, over the past 10 years COPD (Chronic Obstructive Pulmonary 

    Disease) – 6% of all respiratory diseases) and “Running Nose” (an upper respiratory tract condition) have been the relatively faster growing respiratory diseases. COPD that is driven more by external environment (e.g. pollution, biomass fuel) & lifestyle driven factors (e.g. smoking, burning tobacco) has been the key disease driving the growth in respiratory diseases in India. COPD is predominantly an urban phenomenon with metro and tier 1 cities seeing approx. 90% cases, and its occurrence is majorly seen across male patients above 51 years. 

    Specialist Dr segments such as Chest Physicians, Cardiologists, Diabetologists & Consulting Physicians treat COPD patients with anti – asthma solids, anti-asthma inhalers, & nebulization – in descending order of preference. Cough preparation and antibiotics are also used. As per the SMSRC report, since 2014 there has been a 1.3x growth in the usage of Anti-Asthma Inhalants + Nebulization, as compared to Anti Asthma tablets in the management of COPD. Inhalants/Nebulization generally considered a superior and more effective treatment choice, still has immense potential to continue to provide increasing relief to a growing number of COPD patients, given they are still the second ranked category to anti-asthma solids.

    The incidence of Asthma, a more genetic condition contributing 10% of all respiratory diseases, has seen a more moderate increase over the past 10 years. Younger patients below 10 years constitute the largest chunk of asthmatic patients, although an increasing proportion of elderly patients are also seen being diagnosed with asthma as compared to 10 years ago (35% above 41 years of age in 2014 vs 41% above 41 years of age in 2024). 

    Chest Physicians, Cardiologists, and Consulting Physicians generally prefer anti – asthma solids followed by inhalers/nebulizers. Since 2014, the growth in the usage of inhalers/nebulizers has been 1.2x, as compared to that of Anti Asthma tablets. However, inhalers are still second preference to anti-asthma tablets amongst Chest Physicians, Cardiologists and Consulting Physicians. Unlike COPD though, given the younger age cohort in asthmatic patients, Paediatricians are also an important segment but the usage of inhalers are much lesser, with anti-asthma tablets & syrups and cough preparation being a more preferred mode of treatment.

    Surat (Gujarat) (India) July 10 : ‘Surat Obstetric and Gynecological Society’ in the city  arranged a three-day “FOGSI Presidential Conference”  from July 5th to 7th, 2024, at Hotel Le Meridien on Dumas Road,surat

    This conference featured 90 expert doctors specializing in various fields, conducting workshops, panel discussions, and seminars to provide information and skills on useful modern and scientific methods in women’s healthcare management. Over 500 gynecologists from across the country, including Gujarat, Mumbai, Pune, Kochi, Kerala, Delhi, Chennai, Hyderabad, Uttar Pradesh participated in this conference.

    When providing information about this conference, Dr. Praful Doshi, the President of FOGSI Surat Chapter, mentioned that in the field of medical sciences, advancements and experiments are constantly happening to facilitate patient care quickly and effectively., The purpose of this national level conference was to provide information about such useful modern, technology-based, and scientific methods in the management of women’s healthcare. During the conference held from July 5th, separate workshops were organized on various topics. Among them, (1) high-definition live laparoscopic surgery was transmitted to Hotel Le Meridian from Universal Hospital on Ring Road, Majura Gate.(2) Another topic covered methods for controlling and managing Postpartum Hemorrhage (PPH) occurring after childbirth through training using models. (3) third topic covered ultrasonography by doctors from Hyderabad and Chennai who demonstrated how to differentiate uterine diseases.

    Dr. Doshi further mentioned that a few years ago in the country, the mortality rate during childbirth was approximately 800 to 900 per 100,000 patients. With advancements in medical science and the availability of modern technology for healthcare, this number has successfully been reduced to 150-200 per 100,000 patients. This workshop proved to be very useful and informative for gynecologists and undergraduate/postgraduate students, providing valuable guidance.

    The conference also featured  organising chairperson Dr.Nimish Shelat, Organizing Secretary Dr. Arpit Vachhani, Dr. Darshan Vadekar, Dr. Rozy Ahya, Treasurer Dr. Dipti Patel, Dr. Narendra Vaghela, Dr. Anita Shah, Dr. Prakash Bhatt, and Dr. Asha Bhatt among other doctors who provided various insights.

    It is noteworthy that FOGSI is the largest organization globally with around 44,000 member doctors. The Presidential Conference of FOGSI held in Surat was attended by prominent figures including FOGSI president Dr. Jaydeep Tank, Secretary Dr. Madhuri Patel, and South Asian Federation of Obstetrics and Gynecology (SAFOG) Chief Dr. Shyam Desai.     Beyond standard measures of prevalence and usage, Industry Leaders and Health Experts call for a nuanced approach to solving for Tobacco impact

    New Delhi (India), July 4: To commemorate Doctors’ Day, the Times Now Doctors Day Conclave brought together leading medical professionals to celebrate their contributions and address pressing health issues facing India. One such critical issue is the devastating toll of tobacco use. While existing policies aim to curb tobacco consumption, the human and economic cost remains significant. 

    Current national tobacco control policies haven’t yielded the desired results. Millions of lives are lost each year to tobacco-related diseases, placing a crushing burden on India’s healthcare system. The panel discussion on “India needs a new and comprehensive approach to combating addiction led NCDs” at the Conclave highlighted the urgent need for a fresh and comprehensive approach to combat addiction-driven NCDs, particularly the root cause – nicotine addiction.

    Yugal Sikri, Vice Chairperson & Managing Director of RPG Life Sciences, emphasized the gravity of the situation, stating, “Addressing non-communicable diseases is not just important but critical, demanding a fresh and comprehensive approach to combat addiction driven NCDs. The rise of these NCDs linked to addiction is alarming, with tobacco alone responsible for one in six NCD-related deaths.”

    The discussion ignited with a thought-provoking question posed by Yugal Sikri: Is it nicotine itself, or the way it’s delivered, that poses the greater threat? 

    Dr. Sequeira, a leading public health figure, built upon this by urging a clear distinction between nicotine and tobacco. He called out the issue of misinformation surrounding nicotine, even among healthcare professionals. Nicotine is addictive, but the nicotine in cigarettes, bidis etc is not directly responsible for the cancer, lung disease, and heart disease that kill hundreds of thousands of Indians each year.  Yes, it got them all addicted and kept them addicted for the long term. But it’s the other chemical compounds in tobacco, and in the smoke created by setting tobacco on fire, that directly and primarily cause the illness and death, not the nicotine. So we need to take a fresh look at nicotine itself, and how the addiction that it causes relates to the potential harm of its delivery mechanism.

    Atish Majumdar, Senior President of Mankind Pharma, pointed out “We are faced with a monumental challenge. Increasing awareness is crucial in tackling the impact of addiction-driven NCDs and promoting harm reduction. Clearly defining addiction is essential. Beyond tobacco, sugar, and salt also play significant roles as contributors to addiction-driven NCDs,” he noted. 

    Health-risk labels are therefore critical for any addiction related NCDs – be it tobacco or food products.

    Dr. Mohsin Wali, Padma Shri Awardee & Senior Consultant in Oncology, Healthcare, Cancer, and Clinical Research at Sir Gangaram Hospital, stressed the importance of prioritizing the future well-being of youth. He said, “It is crucial to prioritize the future and the well-being of our youth. Now is the opportune moment to confront addiction-driven non-communicable diseases (NCDs), particularly the increasing prevalence of smoking among younger demographics.”

    The panel discussion acknowledged that the need of the hour therefore is a more nuanced approach to characterizing the overall impact of the tobacco market—something better than tobacco-use or prevalence. A new approach of ‘harm weighting’ or ‘risk grading’ in characterizing tobacco use, basically creating an index of tobacco use that takes account of the differences in risk between different products. Traditional cigarettes and bidis reside at the most hazardous end of this spectrum, while medicinal nicotine products (like NRTs) fall on the safer side. By recognizing the risk index, the conversation can shift. Instead of coping with tobacco control, we would be solving for tobacco control. 

    Dr. Rohan Savio Sequeira, Consultant Cardio-Metabolic Physician and Specialist in Non-Invasive Cardiology, Diabetes & Endocrinology, and Hon. Consultant Physician to the Governor of Maharashtra, emphasized the need for educating medical professionals and pharmaceutical companies on addressing the real root cause. “Patients come and ask us on how do I quit? We as medical professionals are at the forefront of this, therefore we need to fully comprehend what ‘harm’ and ‘harm reduction’ means. 

    Dr Sequeira also called out the inadequacy of Nicotine Replacement Techniques by pointing out that ‘Data suggests that NRTs have an efficacy rate of just 7%. The challenge primarily lies in addressing nicotine delivery systems. It is crucial to prioritize education as part of the National Health Policy. This includes educating rural physicians on the dangers of addiction and effective strategies to minimize harm,” he said.

    Dr. D. Dhanuraj, Founder-Chairman, Centre for Public Policy Research (CPCR) elaborated further on nicotine delivery systems, “Over the past decade, reducing harm from tobacco use has become crucial, prompting us to involve stakeholders in accelerating awareness efforts. Despite increased taxes on tobacco products, usage rates have continued to rise. Alternative nicotine delivery systems present a viable option for reducing tobacco use and merit further exploration.”

    The Conclave concluded by calling for a novel public health strategy that embraces risk-based profiling of tobacco products. This strategy would acknowledge the complexities of nicotine dependence and prioritize the well-being of the Indian population. A family and population-focused approach to reducing tobacco-caused disease and death must start from the premise that, as far as tobacco is concerned, the bigger problem is the delivery system. By leveraging scientific advancements, fostering open discussions among healthcare professionals, and implementing innovative policies, India can chart a new course in the fight against tobacco-related NCDs.
