  • What Happens after Laparoscopic Hernia Surgery?

    New Delhi [India], July 15: Laparoscopic hernia surgery is an advanced minimally invasive approach that has a very high success rate as compared to open surgery. “There are different treatment options for hernia, but if you are seeking effective treatment that guarantees quick recovery with no complications, laparoscopic surgery is the best resort,” says a senior hernia specialist at Pristyn Care.

    Typically, laparoscopic surgery for hernia repair is performed under general anesthesia. It is a key-hole procedure that involves making a small incision near the belly button and inflating the abdomen with CO2 gas. This elevates the abdomen and provides a better view of the pelvic organs for the surgeon.

    The surgeon then pushes the hernia back into place with the assistance of a camera-fitted laparoscope. A mesh is occasionally inserted to hold the surrounding muscles in place. This prevents the organ from perforating through the weakened muscles again.

    While recovering after a laparoscopic hernia operation, the primary objective is to manage pain, promote wound healing, and regain abdominal muscle health.

    Recovery from laparoscopic surgery is different for each patient. Each patient recovers at his own pace.

    What to Expect After a Laparoscopic Hernia Surgery

    Postoperative pain and discomfort: When a patient awakens from anesthesia in the recovery room, they may experience some discomfort. It is necessary to ensure that the pain is managed appropriately.

    Restricted physical activity after surgery: There are no established guidelines for activity following laparoscopic procedures. In some cases, doctors usually recommend not to drive for two weeks. Drive only when you are confident in your ability to stop the vehicle in an emergency; otherwise, have someone drive you. Swimming and bathing are prohibited as well. If you are able, you may take the stairs. When beginning routine exercise following surgery, use common sense and gradually increase your activity level.

    Limited Sexual activity: Intercourse should be avoided for at least two weeks after a hernia operation. The period can also vary depending on the extent of the procedure. Sexual activity is typically not restricted following laparoscopic inguinal hernia repair. In such cases, the patient is likely to experience discomfort and pain for a week or two around the incision site.

    In male patients, the scrotum may be discolored, tender, or noticeably swollen following surgery. The swelling usually subsides within a week or two and the patient can resume normal activities and sexual intercourse. Indulging in sexual activity before proper healing can result in complications.

    Abdominal discomfort / bloating: After laparoscopic hernia surgery, some degree of abdominal pain and swelling can be expected. This occurs due to intestinal distension and resolves over time. Gas pains in the intraperitoneal cavity are caused by gas trapped outside the intestines but within the abdominal cavity.

    Nausea: It is a very common side effect of laparoscopy. Anesthesia drugs are the primary reason for nausea following hernia surgery. In addition, pain medication or antibiotics can also cause nausea after the first 24 hours of the surgery. Consult your surgeon if the problem persists for more than 2-3 days.

    Pain in the shoulder: Carbon dioxide gas used to artificially boost the abdomen during laparoscopy has the potential to irritate the phrenic nerve. This is due to trapped carbon dioxide gas against the diaphragm (breathing muscle). This irritation feels like pain and discomfort in the chest and up into the shoulder region, a condition referred to as “referred pain.” The discomfort may occur when taking deep breaths. This type of pain after hernia surgery can last for several days. It will eventually resolve on its own, but walking and moving around can assist. Massage, cold/heat packs, and non-steroidal anti-inflammatory drugs (such as ibuprofen or paracetamol) frequently provide the quickest relief.

    If the pain persists or worsens, it is necessary to rule out other possible causes of chest pain, such as heart or lung problems.

    Sore throat: Some patients may experience a sore throat as a result of the tube placed during anesthesia. Lozenges for the throat or warm tea help alleviate the discomfort, which should resolve within a few days.

    Minor bruises: Bruises may develop at the incision sites in some patients. This is because of “the trocars,” a type of plastic sleeve that is capable of cutting tiny vessels just beneath the skin. These usually self-resolve. Pain in the area of the incisions is common and will subside over several days. Due to the nerves being cut, you may experience “pins and needles” at the incision site. These nerves will eventually heal.

    In addition to managing the above-mentioned side-effects of hernia surgery with the instructions of the doctor, the patient also should ensure the following things:

    • Proper wound care: Doctors advise the patients to keep the wounds dry and clean. There is no need for special creams or ointments. The incisions are closed with a suture placed beneath the skin that dissolves naturally. The sutures are covered with a surgical glue called “derma-bond.” This covers the incision and remains in place for at least two weeks. Remove the glue with soap and water and gentle scrubbing only when the doctor allows it.
    • Careful while taking shower: Doctors allow you to take shower for only 36 hours following the surgery. The individual can remove any band-aids from the incisions before showering. Also, refrain from itching and using any harsh soaps around the incision site as these can lead to rupturing of sutures and inflammation.

    It is normal to notice small pieces of tape (referred to as steri-strips) adhered directly to the skin. It is acceptable to get these small tapes wet while showering. The tapes will begin to peel away from the ends 7–10 days after surgery. At this point, they have served their purpose and the patient may peel them completely off gently. Remember, for two weeks, no baths, pools, or hot tubs to avoid any infections.

    • Avoid medication overdose: Prescription medications for postoperative pain should be used sparingly because they have the potential to cause constipation. Narcotic analgesia can occasionally aid in sleep.
    • Use a heat pack: It is safe to apply a heat pack to the lower abdomen. Coughing can be a source of discomfort. Using a pillow to support your abdomen while coughing can be beneficial.

    What to do in case of severe postoperative problems?

    It is quite normal to experience some discomfort and pain for a few days after hernia correction surgery. These usually resolve themselves and are nothing to worry about.     Coimbatore (Tamil Nadu) [India], July 8: All India Poultry Breeders Association (AIPBA) organized a webinar to discuss diseases and issues in the poultry sector and to bring it to the notice of the officials of the Animal Husbandry Department. The webinar was aimed to find a proper solution to make the poultry industry disease-free and help India to be the world leader in chicken and egg production.

    There was an important discussion on this subject between many industrialists of the poultry industry and senior officers of the Animal Husbandry Department.

    Mr. Gulrej Alam, Secretary, AIPBA and Director, IB Group, was the host of the webinar. The moderator of the webinar Prof. P.K. Shukla, Dean & Registrar DUVASU Veterinary University, Mathura, and Shri Vijay Sardana Advisor, AIPBA conducted the discussion smoothly.

    In the webinar, AIPBA Chairman and IB Group’s Managing Director Mr. Bahadur Ali welcomed all the panelists and officers of the Animal Husbandry Department including Dr. Praveen Malik Animal Husbandry Commissioner, Animal Husbandry Department Mr. OP Chaudhary Joint NLM PC Animal Husbandry Department Mr. Upmanyu Basu Joint Secretary LH Animal Husbandry.

    He shared the problems caused by diseases in front of the officials, he said, “The last 2 years have been very difficult for the poultry industry and the Animal Husbandry Department has also provided all possible help to the poultry industry, but it has become very important to find a permanent solution to the diseases of the poultry so that it becomes possible to prevent the loss of 20-25 thousand crores every year. Vaccines can be a way to avoid the damage caused by the disease.”

    Dr. Praveen Malik, Commissioner of Animal Husbandry Department, furthering the discussion said that two-three diseases like influenza and microplasma trouble the poultry every year and our effort is also to eliminate the disease instead of controlling it.

    The Joint Secretary, Animal Husbandry Department, Shri OP Choudhary NLMPC, and Shri Upamanyu Basu AHAL also agreed on the need for the vaccine.

    Shri Upamanyu Vasu said that vaccination is very important because the vaccine is the most effective way to destroy any disease from the root, but along with vaccination, biosecurity and hygiene are also necessary, after discussing with the poultry industry, all steps should be taken by adopting the scientific method. So it will prove to be a positive step for the growth of poultry.

    Raising important issues of Poultry, Mr. Balram Singh Yadav, MD, Godrej Agrovet Limited said that Poultry Industry needs to think from tomorrow’s perspective also so that the image of the Poultry Industry can be improved further. In the last one or two years, the poultry industry has suffered a loss of thousands of crores because whenever any virus or disease comes, it has the biggest impact on our industry.

    During this discussion, along with all the office breeders of the association, about 1500 poultry farmers from the app and 3572 poultry farms connected to Facebook and YouTube in social media who put their questions in front of the officers of the Animal Husbandry Department so that these problems could be resolved.

  • POST-COVID patient with lung complication undergoes successful surgery – First in north Kerala (Malabar)

    Kozhikode (Kerala) [India], June 1: A 52 years old gentleman hailing from Beypore presented to PVS hospital, Calicut, Kerala on the night of 22 May 2021 in a critical condition with high fever, left chest pain, cough, and severe breathlessness. He had received treatment elsewhere for the past 5 days with no relief. In his medical history he revealed that he had contracted COVID a few months earlier for which he took treatment, and before that, he was perfectly healthy and was leading an active life and attending to his construction business. Although he became negative for COVID, he never fully recovered and was visibly tired. He was admitted to the ICU under the care of the physician Dr. Sruthi Haridas. Despite Oxygen administration, his saturation level was only around 92 %. He was diagnosed to have pneumonia and fluid in the left chest. His respiratory distress worsened despite powerful antibiotics. Non-invasive ventilation and needle drainage of fluid were done with little success.

    As his condition rapidly deteriorated, expert opinion was sought from Senior Pulmonologist and Intensive Care Consultant Dr. Ajit Bhaskar who suspected the possibility of pus in the chest cavity. CT scan revealed pneumonia in the left lung along with a fluid-like image causing complete lung collapse. The patient was living only on his right lung which too was partially damaged due to COVID.

    Noted Thoracic Surgeon Dr. Nasser Yusuf was called in to consider the possibility of surgery. The initial attempt by tube drainage was unsuccessful as the pus had become rock solid and trapped the left lung preventing it from expanding.

    The challenge was operating on a COVID Lung. Although COVID had come to India on January 27, 2020, and subsequently became a pandemic, only of late the sequelae – long-term complications are being recognized which could be devastating. A search of the world literature on Surgery of lung complications revealed very little. The main fear is surgery as well as invasive ventilation in an already damaged lung as they could further compromise the lung function. The gravity of the situation was clearly explained to the family. Having little other option, the family consented.

    On 6 June 2021, he was taken up for urgent surgery. The finding was of fleshy and hardened pus amounting to a kg and forming a thick layer around the lung. This was removed with difficulty and the trapped lung was freed by stripping the thickened layer around the whole lung in a manner similar to peeling wallpaper off a wall. This expectedly caused laceration injury and bleeding from the lung. The entire procedure lasted 6 hours. The patient was then carefully taken off the ventilator by the Anesthetists Dr. Nalini Warrier and Dr. Anu Thomas.

    Postoperative improvement in lung function was remarkable and his Oxygen saturation slowly returned to normal levels. The patient was under the care of Dr. Abdul Rehman, Pulmonologist, and discharged successfully without oxygen exactly 1 week after surgery.

    Post COVID syndromes usually affect mainly the lung, heart, and brain. Lung fibrosis is known to occur and last as long as up to 6 months. Some patients with lung complications following COVID such as Pneumothorax, Ruptured Lung (Bronchopleural fistula), Cavities, Bullae, Bronchiectasis, lung infarct, empyema, and entrapment may benefit from surgery says Dr. Nasser Yousuf MS., MCh, FIACS, FICC who is also trained in UK.
