  • Diphtheria in Pediatric Populations- Understanding the Recent Outbreaks in India

Diphtheria in Pediatric Populations- Understanding the Recent Outbreaks in India

Bangalore (Karnataka) [India] October 16: Diphtheria, a highly contagious bacterial infection caused by Corynebacterium diphtheriae, has historically been a serious public health concern, particularly affecting children. While diphtheria became less common with the widespread use of vaccines, recent outbreaks in India have raised concerns. This article aims to explain what diphtheria is, how it can be diagnosed and treated, and why the recent resurgence has occurred. It will also outline the roles of the government, private healthcare, and communities in controlling this potentially life-threatening disease.

    What is Diphtheria?

Diphtheria primarily affects the throat and upper airways and produces a toxin that can cause severe complications, including damage to the heart, kidneys, and nervous system. It manifests with symptoms such as fever, sore throat, difficulty breathing, and the formation of a thick, gray membrane over the throat and tonsils. Children are particularly vulnerable to severe cases if untreated. The disease is spread through respiratory droplets, coughing, sneezing, and direct contact with contaminated surfaces.

    Diagnosis and Treatment

Diphtheria can be diagnosed through a physical examination, where a doctor may notice the characteristic gray membrane. A throat swab is taken to identify the presence of Corynebacterium diphtheriae. Laboratory tests confirm the diagnosis, and in some cases, it is crucial to begin treatment even before confirmation to prevent complications.

Treatment for diphtheria involves the administration of diphtheria antitoxin, which neutralizes the toxin produced by the bacteria. Antibiotics such as penicillin or erythromycin are given to eliminate the bacteria and prevent further transmission. Early diagnosis and prompt treatment are essential to reduce complications and fatalities, particularly in pediatric patients.

    Prevention of Diphtheria

Vaccination is the cornerstone of diphtheria prevention. The diphtheria vaccine, often administered as part of the DPT (diphtheria, pertussis, and tetanus) combination, is highly effective in preventing the disease. It is part of the routine immunization schedule for children in India, given at 6, 10, and 14 weeks of age, followed by booster doses.

Despite the availability of vaccines, gaps in vaccination coverage can occur, leading to outbreaks. This highlights the importance of maintaining high vaccination rates, timely booster doses, and continued public awareness campaigns about the benefits of immunization.

Why Has India Seen Recent Outbreaks?

India’s recent diphtheria outbreaks can be attributed to several factors. A review of published studies on diphtheria in India indicates that the re-emergence of diphtheria as a public health problem is mainly due to two reasons:

    1. Low Coverage of Diphtheria Vaccination, Especially Booster Doses: Although the primary vaccination schedule covers most children, the coverage of booster doses is significantly lower. Many children do not receive booster shots at the recommended ages of 5 and 10-12 years, which leaves them susceptible to infection as their immunity wanes.
    2. Decline in Immunity: Incomplete primary vaccination or failure to receive booster doses leads to a decline in immunity over time. As immunity decreases, children and even adolescents are at risk of contracting diphtheria. This phenomenon has been exacerbated by the disruption of vaccination services during the COVID-19 pandemic, which led to missed doses for many children.
    3. Compromised Vaccination During the COVID-19 Pandemic: The pandemic disrupted routine healthcare services, including vaccination programs. With healthcare systems overwhelmed by COVID-19, many children missed their scheduled vaccinations, leaving them vulnerable to vaccine-preventable diseases like diphtheria. Studies have shown a decline in routine immunization during the pandemic, which could explain the current surge in cases.
    4. Antibiotic Resistance: There is growing concern about antibiotic resistance in the treatment of diphtheria. Corynebacterium diphtheriae has shown resistance to certain antibiotics, making treatment more difficult and increasing the risk of transmission. While research is ongoing, this is a worrying trend that underscores the need for careful antibiotic stewardship and continuous monitoring of bacterial strains.

The Role of the Government and Private Sector

To combat the resurgence of diphtheria, both the government and the private healthcare sector must take active steps:

    • Strengthening Immunization Programs: The government must ensure that vaccination programs are reinforced, particularly in areas that have experienced interruptions during the pandemic. Initiatives such as catch-up vaccination drives, targeting children who missed doses, can help bridge the gap.
    • Public Health Campaigns: Awareness campaigns about the importance of timely vaccinations are critical. This is particularly important in rural areas, where vaccination rates tend to be lower, and misinformation about vaccines can spread.
    • Monitoring and Surveillance: The government and private hospitals need to implement robust disease surveillance systems to track diphtheria cases. This includes quick identification of outbreaks and early intervention to prevent wider transmission.

    What Can Communities Do?

Communities play a vital role in preventing diphtheria outbreaks. Parents and caregivers must ensure that children receive all recommended vaccinations on time. Schools and childcare centers should work closely with local health authorities to promote vaccination and ensure that children are up-to-date with their immunizations.

Additionally, during outbreaks, communities should practice good hygiene, such as regular handwashing, wearing masks in crowded places, and avoiding contact with sick individuals. Timely reporting of symptoms and seeking medical attention are crucial to controlling the spread of the disease.

    The Role of Healthcare Providers

As healthcare providers, pediatricians and family doctors have a responsibility to educate parents about the importance of vaccines and ensure children receive their scheduled doses. We must be vigilant in diagnosing diphtheria early, providing appropriate treatment, and reporting cases to health authorities. In addition, healthcare professionals should stay informed about the latest research on diphtheria treatment, particularly in light of emerging antibiotic resistance.

The resurgence of diphtheria in India is a reminder that even diseases once thought to be under control can re-emerge if vaccination efforts wane and treatment becomes less effective. Through coordinated efforts between the government, healthcare providers, and communities, India can work to prevent further outbreaks and protect children from this dangerous disease. We must strengthen immunization programs, monitor antibiotic resistance, and remain vigilant in our public health efforts to ensure that diphtheria does not become a major health threat again.

  • Pediatric Asthma Emergency: Managing Acute Exacerbations Amid Rising Pollution

Pediatric Asthma Emergency: Managing Acute Exacerbations Amid Rising Pollution

Bangalore (Karnataka) [India] October 16: Dr Shiv Tej N and Dr Venugopal Reddy I, members of the Pediatric Team at Ovum Woman and Child Hospital, Bangalore, explain everything you need to know about Asthma in children.

    Why is Asthma Important?

Asthma is one of the most common chronic illnesses in children, affecting about 14% of children globally according to the World Health Organization (WHO). It is a serious condition that can lead to life-threatening emergencies if not managed properly. With increasing pollution levels, more children are experiencing severe asthma symptoms, making awareness and management of this condition crucial.

    Understanding Asthma

Asthma is a condition that causes the airways in the lungs to narrow and swell, producing extra mucus. It can be triggered by a variety of factors, such as:

    • Air pollution (like particulate matter and nitrogen dioxide)
    • Viruses (such as RSV, rhinovirus, or influenza)
    • Allergens (dust mites, pet dander, pollen)
    • Emotional or physical stress

These triggers cause the muscles around the airways to tighten (bronchospasm), and the lining of the airways becomes inflamed, which leads to symptoms.

    Symptoms of Asthma

Early recognition of asthma symptoms can help manage the condition and prevent emergencies. Look out for the following:

    • Common symptoms.
    • Wheezing or coughing (especially at night)
    • Shortness of breath
    • Fatigue or tiredness
    • Recurring cough
    • Severe symptoms:
    • Extreme difficulty in breathing
    • Trouble speaking or doing physical activities
    • Anxiety or confusion
    • Low oxygen levels

    Managing an Asthma Attack

Asthma attacks, or acute exacerbations, can escalate quickly, so prompt treatment is essential. The goal is to relieve symptoms rapidly, prevent the attack from worsening, and reduce the need for hospitalization.

    • Mild attacks can be treated with inhaled short-acting beta agonists (like albuterol) and oral corticosteroids.
    • Moderate attacks may require the addition of ipratropium bromide or intravenous medications like magnesium sulfate.
    • Severe attacks might need oxygen therapy, intravenous corticosteroids, and possibly mechanical ventilation.

    How Pollution Impacts Asthma

Pollution is a major trigger for asthma attacks in children. The developing lungs of children are more vulnerable to damage, and their higher breathing rate increases their exposure to harmful pollutants like:

    • Particulate matter (PM2.5): These tiny particles penetrate deep into the lungs.
    • Nitrogen dioxide (NO2): This gas increases inflammation in the lungs.
    • Ozone (O3): Ground-level ozone can damage lung tissue.

Impact of Frequent Asthma Attacks on Children

Repeated asthma attacks can seriously affect a child’s health, both physically and emotionally:

    • Physical Effects:
    • Decreased lung function and increased risk of respiratory failure.
    • Permanent changes in the airway structure, leading to worsened symptoms.
    • Weakened immune system, making children more susceptible to infections.
    • Emotional and Psychological Effects:
    • Anxiety and stress due to fear of attacks and hospital visits.
    • Social isolation and depression as children may miss school and playtime.
    • Sleep disturbances and concentration difficulties.
    • Social Impact:
    • Missed school days, affecting education and social development.
    • Limited physical activity and social interactions.

    Preventing Asthma Attacks

While asthma cannot be cured, attacks can be prevented with proper management:

    • For Families:
    • Educate children and caregivers about asthma and its triggers.
    • Ensure regular use of prescribed medications and adherence to treatment plans.
    • Avoid known triggers and regularly monitor symptoms.
    • For Healthcare Providers:
    • Regularly monitor asthma control and adjust treatment when necessary.
    • Develop personalized management plans for each child.
    • Collaborate with caregivers, schools, and community resources to support children with asthma.

Effective asthma management empowers children and families to take control of their symptoms. By understanding the condition, using medications properly, and making lifestyle changes, children with asthma can lead healthier lives.

Dr Anurag Awasthi, Director- Orthopedics & Joint Surgery, Kimaya Healthcare, Gurgaon, Sr. Consultant (Visiting) – Artemis Hospital; C K Birla Hospital, Gurgaon

Arthritis involves the progressive damage to the cartilage protecting the bones in joints like the knee. This condition can arise from inflammatory arthritis (like rheumatoid arthritis), uric acid crystal deposition, previous injuries, or age-related wear and tear (osteoarthritis).

Increasingly, we see patients aged 25-35 with cartilage issues such as chondromalacia patellae. If untreated, these conditions can lead to more extensive arthritis, resulting in significant pain and limitations. Even healthy individuals in their 50s and 60s may experience knee pain and stiffness.

Promising treatments, like orthobiologics (PRP or BMAC derived from the patient’s own blood or bone marrow), can slow cartilage damage and potentially help patients avoid surgery.

Dr Anurag Awasthi, Director of Orthopedics & Sports Injury at Kimaya Healthcare, emphasizes the importance of educating oneself, adopting an active lifestyle, and consulting an orthopedic specialist when needed. For advanced arthritis treatment options includes Joint Preservation Surgery using key hole technique (Arthroscopy); of for Grade IV Arthritis, a joint replacement surgery can be done to improve function. Dr Arvind B. Goregaonkar is Professor and Head of the Orthopaedic Department at LTM Medical College and Hospital, Sion, Mumbai

“Arthritis is a condition that impacts not only the joints but also overall quality of life. Managing arthritis effectively requires a holistic approach. Along with medical treatments, regular low-impact exercises like walking, swimming, and yoga can significantly improve joint mobility and reduce pain. A balanced diet rich in anti-inflammatory foods such as omega-3 fatty acids, fruits, and vegetables also plays a crucial role in managing symptoms. For patients with early-stage knee arthritis, a Partial Knee Replacement (also known as Unicondylar Knee Replacement or UKR) is a logical option. In this specialized surgery, only the arthritic portion of the knee joint is replaced, leaving the healthy part untouched. UKR is ideal when the medial half of the knee joint is affected, and approximately 40% to 50% of patients who undergo Total Knee Replacement (TKR) could be managed with UKR if they opt for surgery earlier. This minimally invasive procedure involves a smaller incision, minimal blood loss, and faster recovery, with implants lasting 15-20 years, making it particularly beneficial for younger patients,” explains Dr Arvind B. Goregaonkar, Professor and Head of the Orthopaedic Department.

Dr Anuj Nigam, Consultant Orthopedic Surgeon, Bhopal, Ex Surgeon AIIMS, Ex Surgeon ESIC Hospital, Trauma & Joint Replacement Specialist

Dr Anuj Nigam, a Consultant Orthopedic Surgeon based in Bhopal and an esteemed former surgeon at AIIMS and ESIC Hospital, specializes in trauma and joint replacement. He explains that arthritis is a progressive disease affecting joints, characterized by gradual damage to joint cartilage and bones. This condition is categorized into four stages, grouped into two grades:

Low Grade Arthritis (Stages 1 & 2) typically begins around the ages of 40 to 50. In these early stages, the cartilage covering the joint sustains damage, leading to a narrowing of the joint space compared to a healthy joint.

High Grade Arthritis (Stages 3 & 4) signifies a more severe condition where the cartilage is completely lost, resulting in significant bone damage and no remaining joint space, often with bone fragments surrounding the joint.

To prevent arthritis, individuals should monitor symptoms such as pain on the inner aspect of the knee, difficulty in walking long distances, climbing stairs, or squatting. Avoiding activities that exacerbate pain, maintaining a diet rich in calcium and protein, and considering health supplements with collagen and glucosamine are advisable. Treatment options vary: low-grade arthritis may be managed through exercises, physiotherapy, routine changes, and PRP, while high-grade arthritis often requires partial or total joint replacement.

Dr Prof. Puneet Mishra, MS Orthopedics, Fellow Hip Preservation and Hip & Knee Arthroplasty (Germany, Switzerland), Director and Unit Head – Orthopedics, Fortis Hospital (Shalimar Bagh), New Delhi

Forty percent of patients with advanced “bone on bone” osteoarthritis of the knee suffer with arthritis involving only the inner [medial] compartment of the knee and therefore  do not need a total knee replacement.

Oxford partial knee  arthroplasty, also known as microplasty,  is  a revolutionary, precise, surgeon dependent surgery to restore the knee by resurfacing  only the worn out cartilage of  inner compartment of the knee & preserving all the ligaments , healthy outer meniscus and cartilage of the outer knee compartment.

Patients are fully functional post-surgery. Recovery is much quicker and painless than a total knee replacement. They can enjoy all activities like yoga, jogging, dancing and floor level activities with full freedom unlike a total knee replacement.

Outcome of microplasty  has been shown to be excellent in terms of pain relief, swift functional improvement, and patient satisfaction. Studies have reported a success rate of over 95% at 10 years follow-up, with high levels of patient satisfaction , lower complication rates and shorter hospital stays compared to total knee replacement.

Dr Sankalp Mutha – Knee Resurfacing Surgeon at Indigo Hospital, Mumbai

Understanding the specific type of arthritis—be it osteoarthritis, rheumatoid arthritis, or another form—is crucial. Dr Mutha emphasizes the importance of staying active with low-impact exercises, such as swimming and walking, to maintain joint function and reduce stiffness. He also highlights that maintaining a healthy weight can alleviate pressure on the joints, while pain management techniques, including over-the-counter medications and alternative therapies, can enhance overall quality of life.

For some patients, surgical options like knee replacement or resurfacing may be necessary. Dr Mutha encourages the use of assistive devices to ease daily activities.

  • PM Modi’s “Catch the Rain” initiative to turn into a mass movement

PM Modi’s “Catch the Rain” initiative to turn into a mass movement

Surat (Gujarat) [India] October 11: Prime Minister Narendra Modi’s dream project for water conservation through public participation is rapidly turning into a mass movement. “Water conservation is not just a project but a sacred cause for the future generations. Water is not just a resource, but a key issue for the survival of humanity,” PM Modi said during the virtual launch of the “Catch the Rain” campaign on September 6 from Surat’s Indoor Stadium.

In just a month, the campaign has gained momentum across Gujarat, with 8,817 water conservation works initiated through public participation. Of these, over 4,588 projects have already been completed, while 4,229 are in progress and are expected to be finished soon. These projects will significantly increase water storage capacity in the state.

The “Catch the Rain” initiative announced by PM Modi focuses on raising public awareness and ensuring active community involvement in rainwater harvesting. In PM Modi’s words, “Water conservation is a sacred duty and the unprecedented results achieved through public participation will serve as an inspiration for the world in the coming days.”

The Ministry of Jal Shakti has outlined a phased plan to expand the “Catch the Rain” initiative across all states in the country. The campaign will be extended to Rajasthan, Madhya Pradesh and Bihar at a special event planned at Surat’s Indoor Stadium on Sunday. The event will take place in the presence of Union Minister of Jal Shakti, CR Patil, Gujarat Chief Minister Bhupendra Patel, Rajasthan Chief Minister Bhajanlal Sharma, Madhya Pradesh Chief Minister Dr. Mohan Yadav, and Bihar Deputy Chief Minister Samrat Chaudhary.

PM Modi’s water conservation initiative “Catch the Rain”

Prominent industry and community leaders from Rajasthan, Madhya Pradesh and Bihar, who have made Surat their home, will take on the responsibility of advancing the “Catch the Rain” campaign in their home states. These leaders have accepted the responsibility of undertaking four projects for water conservation in almost all villages in every district of their home states. A major announcement in this regard will be made during the event, marking a significant milestone in community-driven water conservation efforts.

It is noteworthy that while launching the campaign, PM Modi emphasised the importance of collectively addressing water scarcity amidst the challenges of climate change. He said that India holds only 4% of the world’s potable water and groundwater levels are rapidly depleting in many regions. He also said that citizens will have to unite for water conservation to deal with this situation, and expressed confidence that with public participation, this campaign will play a pivotal role in addressing these issues.

Following these profound words of PM Narendra Modi, the Ministry of Jal Shakti is expanding the water conservation campaign across the country through public participation. After commencement in Gujarat, preparations for the launch of the second phase are complete.
